Evaluating the Impact of Patient Support Programs on Chronic Disease Management and Treatment Adherence
Abstract
Patient support programs (PSPs) have emerged as critical components in contemporary healthcare delivery, particularly in the management of chronic diseases where treatment adherence remains a persistent challenge. This comprehensive review examines the impact of structured patient support interventions on chronic disease management outcomes and medication adherence rates across diverse healthcare settings. The study synthesizes evidence from multiple healthcare frameworks and evaluates the effectiveness of various support modalities including digital health platforms, peer support networks, healthcare provider coaching, and integrated care coordination systems.
The analysis reveals that well-designed patient support programs demonstrate significant positive impacts on treatment adherence, with adherence rates improving by 15-40% across different chronic conditions when compared to standard care protocols. Diabetes management programs showed particularly strong outcomes, with HbA1c improvements of 0.8-1.2% reported across multiple studies. Cardiovascular disease support programs demonstrated reduced hospital readmission rates by 20-35%, while mental health support interventions showed sustained engagement improvements of 25-50% in treatment continuation.
